In most patients, there is an increased urinary excretion of 5-hydroxyindoleacetic acid , a degradation product of serotonin.
The carcinoid syndrome occurs in approximately 5% of carcinoid tumors and becomes manifest when vasoactive substances from the tumors enter the systemic circulation escaping hepatic degradation.
Carcinoid syndrome occurs when a tumor in the neuroendocrine system is malignant and begins to overproduce certain chemicals, or hormones, causing specific symptoms.

An intestinal carcinoid does not usually cause carcinoid syndrome unless hepatic metastases have occurred because metabolic products released by the tumor are rapidly destroyed by blood and liver enzymes in the portal circulation eg, serotonin by hepatic monoamine oxidase.
